HSC Student Award

In appreciation of your membership and support we would like to offer members who are PDHPE and/or CAFS  teachers in secondary schools the opportunity to present an ACHPER Student Award to the Year 12 student at your school who achieved 1st place in your PDHPE class in 2012 and/or 1st place in your CAFS class in 2012

Please either fill in the details below and fax (9572 8603) back to our office or alternately email achpernsw@achper.com.au or phone the office with the name of the school and student/s and the date of your presentation night.  The certificate will be posted to you as quickly as possible.
